Entry Checklist

Entrants in the MDEA competition should use the following checklists to ensure that they remember to include all materials required for their entry to be processed. Late submission or omission of required materials may result in imposition of a late fee or disqualification of your entry. Entries found to be incomplete after the final entry deadline are subject to disqualification regardless of when the original materials arrived or what entry fee has been paid. Entry fees are not refundable under any circumstances. Submitters should remember to label each item with their company name and the name of the product.

Required Items
 
_______ Completed Entry Questionnaire in a hard-copy version signed by the authorized submitter.
_______ Completed Entry Questionnaire in an electronic (e-mailed or PC-compatible diskette or CD-ROM) version.
_______ Appendices and exhibits referenced in answers to the Entry Questionnaire.
_______ One high-resolution digital photograph—typically the product’s “glamour shot,” without artificial labeling—that clearly depicts the entry (no more than five alternative photos are permitted).
_______ Entry fee as required for the date on which the entry is to be received by Canon Communications. Make checks payable to Canon Communications LLC. If the entry fee will be paid with a credit card, the cardholder must authorize payment by signing and dating the hard copy of the Entry Questionnaire in the spaces provided.

_______ Product data sheets or marketing brochures (20 copies maximum).
_______ Additional photos and diagrams for jury use.
_______ Short video (5 minutes or less) in VHS or DVD format, focusing clearly on the product’s design features and usage
_______ Sample products for jury examination and display. For small or disposable items, up to 50 products may be sent.
_______ Airbill for the return of sample products.
